New Patient Registration
If you have your NHS Medical Card from your previous practice, please bring this with you when you register with the practice. It is not essential to have it to register.
As a new patient you will be required to fill in two forms for each patient who is to be registering. These forms are obtainable from the receptionist and when completed, should include important information on current medication, immunisations, allergies and relevant medical family history. We also include a short questionnaire on alcohol consumption.
As we run a personal doctor service, if you have a preference as to which doctor you would like to be your main physician, please let us know.
Initial Registration Form
If you are just moving into the area and have not yet registered with a local GP, we would invite you to register with us.
To register with a GP, please download and complete the ‘Initial Registration Form’, indicating your preference of surgery and we will be in touch. Please note that to fully register with the surgery, you will need to complete a full registration form and provide 2 forms of ID as soon as convenient.
